TELEDENTISTRY

Keeping our clients and staff safe is a top priority at our office.  Under certain circumstances, our office provides teledentistry as a way to minimize having to travel from your safer-at-home locations and being in contact with others. 

Please call our office at 818-344-1393 to discuss if a teledentistry appointment is appropriate for your situation. Our staff will schedule a teledentistry appointment between you and Dr. Kodama.  

 Prior to your teledentistry appointment with Dr. Kodama, complete and email signed copies* of the following forms to our office at Office@JamesKodamaDDS.com. These forms are required before your teledentistry appointment

• Patient Screening Form - complete and download, sign and date, and email the signed copy* to Office@JamesKodamaDDS.com. 
• Consent for Tele-Dentistry Form - complete and download, sign and date, and email the signed copy* to Office@JamesKodamaDDS.com. 

• If you are a new patient, please download and complete our Dental Registration and Health History Form. Sign and date, then email the signed copy* to Office@JamesKodamaDDS.com.

* If you do not have access to a scanner, pictures of your completed and signed forms are OK to email.
